You do not need to drive to your local Social Security office or wait for an appointment with a Social Security. SSA Form 3368, the Adult Disability Report, is the most comprehensive and, some argue, the most important of the forms that you will fill out when applying for disability benefits from the Social Security Administration. Form 3368 is an 11-section form that you must complete prior to your Social Security Disability interview.
